Key Responsibilities\n

As a Licensed Clinical Social Worker, you will play a pivotal role in the following:

• Develop and coordinate discharge plans in collaboration with Reentry Social Workers to ensure appropriate mental health care following detox.

• Complete thorough documentation for all patient interactions, assessments, and treatment plans using electronic health record systems.

• Facilitate group therapy sessions focusing on mental health support, relapse prevention, and coping strategies, tailoring each session to meet the unique needs of your clients.

• Participate actively in clinical supervision sessions, attending at least once a week to discuss cases and professional development.
\n
• Contribute to mental health rounds and case conferences, where you will engage in collaborative discussions about ongoing patient care.
\n
• Coordinate care with various institutional programs and collaboratives, ensuring access to essential housing and treatment resources for patients.
\n
• May supervise social work interns or student volunteers, providing guidance and support in their professional development.
\n
• Conduct psychological testing as necessary, ensuring comprehensive assessments inform treatment strategies.

The client population you will serve primarily includes individuals experiencing substance use disorders, often accompanied by co-occurring mental health issues such as anxiety, depression, or trauma.
