Key Responsibilities\n

• Conduct ongoing assessments to evaluate and address the specific needs of patients served via telehealth platforms.

• Maintain clear and effective communication among supervisors, direct care staff, family members, and other relevant parties regarding a patient’s progress, utilizing electronic health record (EHR) systems.

• Develop and provide a structured schedule of appointments and follow-ups for monitoring patient progress, ensuring adherence to treatment plans.

• Coordinate home visits or virtual check-ins as necessary, aligning with the requirements for patient care while documenting observations and outcomes through thorough data analysis.

• Implement program plans tailored to patients’ needs effectively, ensuring that therapeutic interventions are carried out consistently.

• Participate in interdisciplinary team meetings to contribute insights and collaborate on comprehensive care strategies.
\n
• Prepare detailed monthly and quarterly progress notes, documenting patient advancements and modifying care plans as required.

• Organize training sessions to ensure that staff members are well-equipped to adhere to program protocols and documentation standards.

• Facilitate follow-up interviews and home visits to ensure ongoing support, adjusting treatment paths as necessary based on patient feedback.

• Act as an effective liaison between the public and the mental health community, promoting organizational goals and enhancing program outreach.

• Contribute to the development of training materials and standard operating procedures to elevate the quality of care provided.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.

• A minimum of two years of relevant counseling experience, preferably in a clinical or telehealth setting.

• Demonstrated understanding of legal and ethical standards in mental health practice.

• Strong interpersonal and communication skills, with an ability to establish rapport with a diverse clientele.

• Proficiency in using EMR software and related technologies for documenting patient interactions and treatment plans effectively.

Preferred Experience\n

• Specialization in family therapy, couples counseling, trauma-informed care, or substance misuse treatment is highly desirable.
