Work Environment

This role will be conducted onsite in the natural and comfortable settings of our patients' homes, primarily in the Immokalee area. Working outside the traditional clinical environment allows our therapists to better understand the unique challenges faced by patients in their everyday lives. Key Responsibilities

• Conduct thorough assessments of patients' physical abilities and limitations, utilizing evidence-based practices to develop individualized treatment plans.
• Implement appropriate treatment protocols for a variety of orthopedic conditions, adapting techniques based on patient responses and progress.
• Collaborate with a multidisciplinary team of healthcare professionals, maintaining clear communication regarding patient evaluations and treatment modifications.
• Document all patient care activities accurately and comprehensively, ensuring compliance with clinical and regulatory standards.
• Provide detailed discharge planning to ensure continuity of care post-therapy, including follow-up recommendations and necessary adjustments to home environments.
• Engage with patients, families, and caregivers to educate them about physical therapy interventions and strategies for maintaining optimal health.
• Participate in ongoing training and development to stay current with best practices in physical therapy.
• Attend team meetings and contribute insights from your clinical experience to enhance the quality of care offered by the organization.

Patient Population / Client Focus

You will primarily work with patients recovering from orthopedic surgeries, injuries, and chronic conditions affecting mobility and function. This may include individuals recovering from joint replacements, fractures, and sports-related injuries. Understanding the psychosocial aspects of patient care and adapting treatment plans to meet these needs is crucial in our home care setting.

Qualifications

• A Bachelor's or Doctorate degree in Physical Therapy (DPT).
• A valid physical therapist license in the state of Florida.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.

Preferred Experience

While a minimum of 2 years of experience in a physical therapy setting is required, candidates with a background in orthopedic rehabilitation or home care settings may receive preferential consideration. Experience with electronic health record (EHR) systems and documentation requirements is also beneficial.

Compensation and Benefits
