Full-Time Home Care Physical Therapist

Position Overview

We are seeking a dedicated and skilled Physical Therapist to join the team at Maury Physical Therapy in Immokalee, Florida. This is a full-time position that emphasizes home care, allowing you to deliver personalized therapy directly within patients' residences. Work Environment

As part of our home care service model, you will work directly in patients' homes, providing a unique opportunity to observe them in their living environments. This setting allows for more personalized therapy interventions, addressing real-life challenges that may affect recovery. You will typically have a 40-hour work week, with variable weekends, ensuring flexibility in care delivery while maintaining a high standard of service.

Key Responsibilities

• Conduct comprehensive evaluations of patients to identify functional limitations and develop individualized treatment plans.
• Implement evidence-based therapeutic interventions aimed at enhancing mobility, reducing pain, and improving the overall physical function.
• Monitor patient progress rigorously, adjusting treatment protocols based on observable improvements and feedback.
• Maintain accurate documentation of evaluations, treatments, and progress reports in accordance with regulatory requirements.
• Communicate effectively with interdisciplinary teams and healthcare providers about patient care plans and continuum of care.
• Engage in community outreach and participate in quality improvement initiatives within the organization.
• Supervise and mentor support staff, fostering an environment of learning and professional growth.

Patient Population / Client Focus

Your primary focus will be on adult and geriatric patients who experience a range of orthopedic and functional challenges due to injuries, surgeries, or chronic conditions. Through your expertise, patients will receive individualized attention and support tailored to their specific needs and health goals.

Qualifications

The ideal candidate will possess a Bachelor's Degree in Physical Therapy from an accredited program and hold a valid state license in Physical Therapy. Strong verbal and written communication skills are essential, as well as proficiency in documentation software and basic computer functions. Additionally, current Basic Life Support (BLS) certification is required.

Preferred Experience

We are looking for candidates with at least 2 years of experience in a similar role, preferably with exposure to home care settings. Experience with electronic health record (EHR) systems and direct patient care coordination will be highly advantageous.

Compensation and Benefits

The compensation for this position ranges from USD 100,000 to 120,000 per year, commensurate with experience.
