Key Responsibilities\n

• Provide intensive case management services to a select caseload of children and families, ensuring personalized care to meet their specific health needs.

• Triage and respond promptly to individual tasks and care needs, facilitating timely interventions.

• Guide families in completing necessary forms and applications for healthcare services and assistance programs.

• Collaborate with families to address social determinants of health (SDOH) impacting their well-being, such as housing, access to food, and transportation.

• Connect families to community resources, leveraging available programs and services to enhance their quality of life.

• Track referrals diligently and provide follow-up interactions to guarantee the continuous delivery of services.

• Support patients in scheduling and attending medical appointments, behavioral health consultations, and specialist visits.

• Manage administrative tasks, including record retrieval, eligibility screenings, and goal tracking within our electronic health record system.

• Coordinate comprehensive care by assisting patients with all necessary healthcare services, ensuring a holistic approach.

• Conduct home and community visits as needed to enhance engagement with vulnerable populations.

• Consult and collaborate with interdisciplinary care teams, including behavioral health professionals, medical providers, and external partners, to develop integrated care plans.

• Document all patient interactions and services efficiently using our electronic health record and secondary databases.

• Participate in internal and external meetings, including case conferencing and administrative discussions, to contribute to team strategies.

Qualifications\n

• An active LCSW license in the state of Pennsylvania is required.

Compensation and Benefits\n

We offer a competitive salary ranging from $80,000 to $90,000, commensurate with experience, in addition to a comprehensive benefits package.
