Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments to evaluate clients' social, emotional, and environmental challenges.

• Develop, implement, and review individualized service plans that encapsulate specific goals, interventions, and progress assessments.

• Provide empathetic counseling and support to individuals and families navigating life-altering circumstances associated with grief and loss.

• Work collaboratively with clients to establish personalized service plans, including linking them with community resources for continued support.
\n
• Implement crisis intervention strategies, conduct risk assessments, and develop safety plans to mitigate immediate challenges faced by clients.

• Foster partnerships with community agencies and organizations to effectively coordinate services and champion advocacy for clients’ rights.

• Engage in community outreach initiatives aimed at raising awareness about hospice services and available resources.

• Participate in team meetings, case conferences, and professional development workshops to continually enhance team cohesion and service quality.

Patient Population / Client Focus\n

This position primarily focuses on clients within a hospice setting, including individuals and families dealing with terminal illnesses and bereavement. You will be instrumental in helping clients and their loved ones navigate the emotional complexities of end-of-life care, offering a supportive and compassionate presence during their most challenging times. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
