Key Responsibilities\n

• Conduct patient intakes and assessments, focusing on diagnostic evaluations and functional assessments to recommend treatment goals in collaboration with primary care providers.

• Deliver brief, focused interventions to individuals requiring mental health support, ensuring interventions adhere to best practices set by Agency and Federally Qualified Health Center (FQHC) guidelines.

• Coordinate with primary care providers to identify patients in need of mental health services and ensure appropriate referrals are made.
\n
• Follow up with patients to monitor progress and adjust treatment plans as necessary to ensure optimal care.
\n
• Develop relapse prevention plans for patients needing ongoing support to maintain stability in their mental health.
\n
• Provide education and resources to patients to empower them in self-management of their mental health and facilitate healthier lifestyle choices.
\n
• Document patient progress and treatment plans meticulously in clinical records, ensuring effective communication with the primary care team.
\n
• Collaborate with clinic management and health care teams to evaluate behavioral health services and suggest areas for program improvement.

• Uphold the Code of Ethical Conduct for Clinical Professionals, demonstrating integrity and professionalism in all interactions.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work primarily with diverse populations, addressing a variety of mental health and psychosocial issues. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

Compensation and Benefits\n

The salary range for this position is competitive, estimated between USD 87,000 and 105,000 annually, commensurate with experience and qualifications.
