Key Responsibilities\n

• Conduct comprehensive assessments to identify patient needs and design individualized treatment plans that address their specific challenges.

• Facilitate effective communication between supervisors, healthcare staff, and family members, updating them on client progress and therapy outcomes.

• Provide an organized schedule of therapy sessions, home visits, and follow-ups to ensure the continuity of care.

• Coordinate and perform in-home visits as required, performing observations that will assist in the evaluation process and treatment plan execution.

• Monitor the implementation of treatment programs, ensuring that they are executed effectively and adjusted as necessary based on patient progress.

• Engage in interdisciplinary team meetings and collaborative discussions to enhance the quality of care provided.

• Document detailed monthly and quarterly progress notes that capture patient developments.

• Design and implement training for staff members to ensure they are equipped to adhere to treatment plans and documentation procedures.

• Facilitate ongoing support through follow-up interviews and home visits, ensuring a continuous assessment of client needs.

• Maintain accurate and organized records of patient care, ensuring that sensitive information is accessible only to authorized personnel.

Qualifications\n
