Key Responsibilities\n

  Communicate regularly with the clinical team and referring physicians to report on patient progress, needs, and any concerns regarding treatment plans.

  Consult with clients' families and support systems to coordinate treatment, discharge planning, and ongoing support, ensuring a comprehensive approach to recovery.

  Maintain accurate and timely documentation, including clinical and progress notes, in accordance with departmental policies and relevant regulations.

  Collaborate with the healthcare team to develop and implement individualized treatment and discharge plans based on psychosocial assessments.

  Assist clients with discharge planning and care coordination, leveraging a network of regional resources to ensure access to necessary services and support systems.

Patient Population / Client Focus\n

Your work will primarily involve clients from various backgrounds facing mental health challenges, including anxiety, depression, trauma-related disorders, and substance use issues. Qualifications\n

  A Master’s degree in Social Work, Counseling, Marriage & Family Therapy, or Psychology is required.

  Current and active state licensure as an LCSW, LMFT, LPC, or equivalent is essential for this position.
