Key Responsibilities\n

The Licensed Mental Health Counselor will engage in the following key responsibilities:

• Conduct thorough assessments of patients' psychosocial needs to determine appropriate treatment plans and referral options. This includes identifying cases needing short-term counseling, crisis intervention, or ongoing case management.

• Support patients in navigating health-related processes, including assisting them with entitlements, supplemental insurance applications, and related documentation to reduce reliance on emergency services.

• Collaborate with pharmaceutical companies and community resources to help patients access medication cost assistance programs, ensuring they receive necessary prescriptions.

• Act as a dedicated patient advocate while liaising with community agencies, ensuring these organizations align with our treatment plans and support pathways.

• Maintain accurate and timely documentation of patient records and treatment outcomes, contributing to the overall quality of care provided by the clinic.

• Coordinate transportation assistance for patients who may need help attending appointments, reinforcing our commitment to accessible healthcare services.

• Participate in clinical meetings and case conferences, sharing insights and collaborating with fellow professionals to continuously improve treatment approaches.

Qualifications\n

To be considered for this role, candidates must possess:

• A minimum of two years of professional experience in a therapeutic setting, demonstrating a strong understanding of clinical practices and patient management.

Preferred Experience\n

While the above qualifications are required, we prefer candidates who have experience specifically working in community health care settings. Compensation and Benefits\n
