Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical conditions and mobility limitations.
• Develop individualized treatment plans based on assessment findings, focusing on restoring movement and functional activities pertinent to daily living.
• Deliver therapeutic interventions using evidence-based practices and monitor patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Document all patient interactions and treatment outcomes to comply with regulatory, payer, and accrediting standards.
• Educate patients and their families on care plans and necessary follow-through programs to ensure continued progress outside the clinic.
• Maintain a clean and safe working environment, ensuring all equipment is in good working order and facility protocols are adhered to.
• Participate in team meetings, providing insights on cases to enhance collaborative care approaches and improve practice procedures.

Patient Population / Client Focus

Our patients come from diverse backgrounds and range from post-surgical orthopedic cases to individuals with chronic musculoskeletal conditions. We focus on delivering personalized care that addresses the unique needs of each patient, facilitating their journey towards recovery and improved quality of life. The therapist in this role will be expected to interact positively with patients, encouraging engagement in their treatment process and fostering trust and motivation.

Qualifications

• Active Physical Therapy licensure in the State of California.
• Completion of an accredited Physical Therapy program.
• National Provider Identifier (NPI).
• Minimum of 2 years of experience in physical therapy, particularly in orthopedic and sports rehabilitation settings.

Preferred Experience

• Proficient in developing custom rehabilitation programs based on a patient's unique capabilities and health history.
• Experience with electronic medical record (EMR) systems for efficient documentation and management of patient care.
• Demonstrated ability to work well both independently and within a multidisciplinary team, contributing to a harmonious and effective workplace.
• Knowledge of the latest evidence-based practices in physical therapy and a commitment to continuous learning and professional development.
• Strong interpersonal and communication skills, particularly in relation to patient education and team collaboration.

Compensation and Benefits

Revitalize PT and Fitness Clinic offers a competitive salary in the range of $100,000 - $125,000 per year , commensurate with experience. In addition, we provide a robust benefits package that includes:
