Work Environment

This position operates within a home care setting, emphasizing a personalized approach to patient health. You will be interacting directly with patients at various locations within the Portsmouth area, empowering them through therapeutic interventions in the comfort of their homes. Key Responsibilities

• Develop and execute personalized treatment plans aimed at preventing disability, alleviating pain, enhancing muscle function, and optimizing patient independence based on each client's unique capabilities.
• Oversee and mentor Physical Therapy Assistants, ensuring their effective implementation of treatment protocols in accordance with the established care plan.
• Conduct comprehensive evaluations of clients' functional capacities through diagnostic and prognostic assessments of muscle, nerve, and joint abilities.
• Assess the home environment and make pertinent recommendations to enhance patients' autonomy and rehabilitation processes.
• Administer therapeutic exercises and treatments, including manual techniques like massage and traction, to aid recovery and improve mobility.
• Evaluate, fit, and adjust mobility devices, ensuring proper recommendations to the supervising physician for any necessary modifications.
• Maintain accurate clinical records including initial evaluations, care plans, progress notes, and discharge summaries to ensure continuity of care.
• Collaborate as an integral member of the interdisciplinary team during case conferences, contributing to the collective strategy for optimal patient outcomes.
• Provide education and support to clients and their families about physical therapy practices and self-care techniques.
• Demonstrate readiness to adapt the treatment regimes based on patient feedback and response, refining care strategies as needed.

Patient Population / Client Focus

The patients you will serve primarily include individuals recovering from surgery, injuries, chronic pain, and those needing rehabilitation due to mobility challenges related to aging or chronic illnesses. You will engage with a diverse client base, tailoring treatment to meet the specific needs of each individual, and playing an essential role in their recovery journey.

Qualifications

• A Doctorate in Physical Therapy from an accredited program.
• Current, valid PT licensure in good standing in the state of Virginia.
• Current CPR certification and valid driver's license with automobile insurance.
• A commitment to clinical excellence, integrity, and an exceptional patient care mission.
• Proficiency in state and federal healthcare regulations.

Preferred Experience

We prefer candidates with at least one year of experience as a Physical Therapist in either an acute care or home care setting. Effective interpersonal communication skills, a collaborative spirit, and a willingness to engage in lifelong learning are essential attributes for success in this role.

Compensation and Benefits

We offer a competitive salary range of $90,000 to $100,000 annually, reflecting your experience and commitment to professional excellence.
