Work Environment

This position is primarily onsite, allowing you to interact closely with patients in their homes. Key Responsibilities

As a Licensed Physical Therapist with East Valley Rehab Center, your responsibilities will include:

• Developing and implementing individualized therapy plans to prevent disability, relieve pain, and restore functionality based on the physician's prescription.
• Conducting thorough evaluations of patients' physical capabilities through diagnostic assessments, examining muscle, nerve, joint, and functional abilities.
• Providing direct physical therapy services in patients' homes, administering therapeutic exercises and involving patients in their own rehabilitation processes.
• Monitoring and documenting patients' responses to therapy, adjusting care plans as necessary, and collaborating with healthcare providers to address any changes in the patient's condition.
• Supervising and instructing Physical Therapy Assistants when they provide treatment, ensuring compliance with state regulations and best practices.
• Engaging in continuous education to stay abreast of the latest methodologies and regulations in home healthcare.

This role requires an empathetic and dedicated individual who enjoys working with diverse patient populations, including elderly individuals, patients recovering from surgery, and those with chronic conditions. Qualifications

The ideal candidate will possess the following qualifications:

• A Doctorate in Physical Therapy, obtained from an accredited institution.
• Current and valid Physical Therapist license in good standing in Virginia.
• Up-to-date CPR certification.
• A valid driver's license and reliable transportation, as travel to patients' homes is required.
• Exceptional interpersonal skills with a demonstrated ability to engage patients and their families effectively.

Preferred Experience
