Key Responsibilities\n

• Serve as a crucial liaison between families and outpatient healthcare providers, ensuring seamless communication and care coordination.

• Participate in discharge and aftercare planning processes, collaborating with interdisciplinary teams to enhance continuity of care.

• Provide therapeutic meal support, helping clients develop a healthy relationship with food in a supportive manner.

Qualifications\n

Compensation and Benefits\n
