Key Responsibilities\n

• Serve as an essential liaison between students, their families, and outpatient providers, ensuring comprehensive care coordination.

• Facilitate communication with insurance companies for pre-certification processes and navigate utilization management to support client access to necessary services.

• Engage in the collaborative development of discharge and aftercare plans, proactively supporting transitions back to school or community living.
\n
• Provide therapeutic meal support in school settings, modeling and teaching students about establishing healthy relationships with food.
\n
• Deliver empathetic, affirming, and culturally competent care that takes into account each client’s unique intersecting identities.

• Utilize various therapeutic modalities and frameworks to assess client needs, set treatment goals, and evaluate progress in collaboration with educational staff.

You will be focusing on a diverse population of students, particularly helping those facing challenges related to addiction, trauma, and other mental health conditions. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Hold a valid LCSW license in New York State.
