Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families to uncover emotional, social, and environmental factors that may affect their treatment and recovery.

• Develop, implement, and oversee individualized treatment plans, utilizing a variety of therapeutic modalities tailored to meet the unique needs of each patient.

• Collaborate effectively with a multidisciplinary healthcare team, engaging patients and families in the care planning process to ensure a holistic approach to treatment.

• Facilitate discharge planning and follow-up to support the patient’s transition from detox to ongoing care, coordinating with physicians and third-party payers to ensure the continuity of service.

• Provide education and counseling to patients and families regarding coping strategies and resources related to their health challenges, including mental health and addiction.

• Utilize crisis intervention techniques as needed, engaging in both brief and long-term individual, group, and family therapy sessions.

• Serve as a consultant to other staff members, sharing expertise in the field of social work and psychosocial oncology practices.

• Participate in an on-call rotation to address after-hours needs, ensuring that patients have access to support at all times.

• Present at educational conferences, demonstrating expertise in psychosocial care and contributing to the advancement of practices within the field.

Qualifications\n
