Key Responsibilities\n

• Conduct ongoing assessments to evaluate the mental health needs of patients and create tailored treatment plans based on their individual circumstances.

• Facilitate communication between supervisors, family members, and other stakeholders regarding each patient’s progress and treatment outcomes.
\n
• Organize and manage a tentative schedule of appointments, home visits, and observations to monitor patient response to treatment.
\n
• Coordinate in-home visits to conduct observations, chart reviews, and staff interviews that adhere to clinical best practices.
\n
• Oversee the implementation of treatment plans, ensuring they are effective and delivered consistently.
\n
• Actively participate in Interdisciplinary Team Meetings and contribute to collaborative care discussions.
\n
• Complete thorough progress notes on a monthly and quarterly basis to document patient advancements.
\n
• Provide ongoing support through follow-up interviews and home visits to ensure continuity of care.
\n
• Maintain accurate records to ensure that patient information is readily accessible to the necessary parties.
\n
• Serve as a liaison with the public and mental health community to foster positive relationships and promote developmental goals.
\n
• Assist in developing training materials and procedures to enhance caregiver competency in implementing treatment plans and documentation methods.

Patient Population / Client Focus\n

The LPC will work with a broad spectrum of clients, including both adults and youth dealing with various mental health issues. Your expertise will contribute to addressing challenges such as anxiety, depression, trauma, and family dynamics, thereby making a significant impact on overall community mental health.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor in the state of Kansas.

• A minimum of 2 years of counseling experience, preferably in a clinical setting.

• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is highly preferred.

• A strong understanding of legal and ethical standards within the field of mental health is essential.

• Demonstrated excellence in interpersonal and communication abilities, with the capability to build rapport with clients from diverse backgrounds.

• Proven ability to work collaboratively with multidisciplinary teams to enhance patient care.

• Familiarity with electronic medical record (EMR) software and related technologies is beneficial.

• Salary range of USD 80,000 to 105,000 annually, commensurate with experience and caseload.
