Key Responsibilities\n

• Conduct holistic assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors that affect their quality of life.

• Collaborate closely with clients and their families to create personalized care plans, addressing individual goals, preferences, and any unique challenges they may face.

• Provide empathetic counseling and emotional support to seniors and their families, actively helping them navigate significant life transitions, including end-of-life decisions.

• Advocate for clients to ensure their rights are upheld and that they have access to community resources and services relevant to their needs.

• Maintain accurate records of client assessments, care plans, progress notes, and other relevant documentation in compliance with industry standards.

Qualifications\n

Compensation and Benefits\n

The compensation for this position ranges between $60,000 to $100,000 annually, based on experience and qualifications.
