Key Responsibilities\n

• Conducting comprehensive assessments to identify clients’ social, emotional, and environmental challenges, ensuring that each assessment is thorough and sensitive to individual needs.

• Developing personalized service plans tailored to clients’ specific goals, interventions, and monitoring their progress effectively.

• Providing individual and family counseling, using evidence-based therapeutic approaches to help clients cope with emotional disturbances and life transitions.

• Offering crisis intervention when needed, performing risk assessments, and creating safety plans that address immediate client concerns.

• Collaborating with other healthcare professionals, community agencies, and organizations to coordinate services and provide comprehensive support to clients.

• Participating in community outreach activities to promote our services and educate the community on available resources.

• Engaging actively in case conferences and team meetings to collaboratively develop solutions for complex client situations.

• Maintaining accurate documentation and case notes in compliance with ethical standards and organizational policies.

Qualifications\n

Compensation and Benefits\n
