Telehealth NP — Transitions of Care, Flexible Hours

Galileo

Galileo Health is seeking a culturally competent Nurse Practitioner to join our Population Health team. You will provide telehealth follow-up for patients recently discharged, focusing on medication reconciliation, symptom screening, and coordinated care across settings.

The role requires SC licensure with potential additional state licenses. Expect 32–36 hours/week, weekend coverage, and a flexible schedule in a remote, digital-first environment.
