Key Responsibilities\n

• Conduct ongoing assessments to identify the individual needs of each patient, tailoring treatment plans accordingly.

• Maintain clear and open communication with supervisors, direct care team members, and family members regarding patient progress and treatment adaptations.
\n
• Develop and share a preliminary schedule with supervisors, outlining appointments, home visits, and observations to track therapeutic engagement.
\n
• Coordinate home visits and virtual check-ins to perform in-depth observations, chart reviews, and interviews that inform care strategies.
\n
• Ensure that the implemented program plans are executed effectively, optimizing therapeutic outcomes.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing insights and strategies based on patient assessments.
\n
• Complete monthly and quarterly progress notes to document the effectiveness of interventions and facilitate a data-driven approach to mental health care.
\n
• Organize and conduct training for staff and team members, ensuring everyone is adept in implementing program plans and maintaining documentation standards.
\n
• Provide ongoing follow-up interviews and virtual site visits to offer continued support for each client's evolving needs.
\n
• Keep meticulous records that facilitate accessibility for interested parties while ensuring confidentiality and compliance with legal and ethical standards.

• Act as a liaison with public and private mental health sectors, enhancing collaborative efforts towards developmental goals.

• Contribute to the development of training materials aimed at refining practices for the benefit of our clients.

Patient Population / Client Focus\n

In this role, you will have the opportunity to work with a diverse client base, including adults, adolescents, and families. You will be instrumental in addressing a broad spectrum of mental health challenges, including anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is mandatory.

• Currently active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Colorado.

• A minimum of 2 years of counseling experience, preferably in a clinical context.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance abuse treatment is highly desirable.

• Strong understanding of mental health legal and ethical standards and practices.
\n
• Exceptional interpersonal and communication skills for building rapport with clients from varied backgrounds.

• Demonstrated ability to effectively collaborate with a multidisciplinary healthcare team.
\n
• Proficiency in using Electronic Medical Records (EMR) software and related technology.

Preferred Experience\n

While not required, candidates with experience in trauma therapy or substance abuse counseling will be preferred. Familiarity with telehealth platforms and virtual service delivery is advantageous, allowing for a smoother transition and effectiveness in patient engagement.
