Work Environment

As a Physical Therapist within our Home Care Division, you will work onsite at patients' residences. This unique setting allows you to create a personalized healing environment that caters to each patient's needs, all while building meaningful relationships. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions and create comprehensive treatment plans tailored to their specific recovery needs.
• Utilize a variety of therapeutic techniques, including exercises, manual therapy, and state-of-the-art equipment, to relieve pain and enhance mobility.
• Regularly assess and document patient progress, adjusting treatment strategies and objectives as necessary to ensure optimal outcomes.
• Educate patients and their families on rehabilitation exercises, assistive devices, and strategies for safe mobility and self-care.
• Actively participate in professional development by attending workshops, engaging in continuing education opportunities, and collaborating with other healthcare professionals.
• Advise patients on appropriate assistive devices and equipment to facilitate their rehabilitation process.

Patient Population / Client Focus

This role focuses on delivering care to a diverse patient population ranging from children to older adults, all of whom may have complex rehabilitation needs due to various conditions, including post-surgical recovery, acute injuries, and chronic illnesses. Your ability to adapt treatment modalities based on age, diagnosis, and individual preferences will be a critical factor in helping patients achieve their rehabilitation goals.

Qualifications

• A Bachelor's degree in Physical Therapy or a related field.
• Active State License in Physical Therapy must be obtained prior to starting.
• Strong written and verbal communication skills to effectively engage with patients, families, and healthcare teams.
• Proficient in using technology and software for patient documentation and communications.
• A flexible, adaptive approach to patient care and a compassionate demeanor.

Preferred Experience

While we strongly prefer candidates with at least 2 years of experience in home care or outpatient therapy settings, we welcome recent graduates who are eager to learn and grow within our organization. Compensation and Benefits
