Work Environment

The Home Health Physical Therapist role is primarily home-based, allowing you to engage directly with patients in their own living environments. This setting enables you to assess their unique challenges and tailor your therapeutic interventions accordingly. You will have a manageable caseload, focusing on delivering personalized care with 30 to 60-minute appointment slots, which ensures that you can devote adequate time to each patient.

Key Responsibilities

• Provide evidence-based orthopedic therapy directly in patients' homes, utilizing an individualized approach tailored to each patient's specific needs and recovery goals.
• Collaborate actively with in-house MDs, OTs, chiropractors, and outpatient PTs to develop integrative care plans that foster optimal recovery.
• Conduct thorough assessments of patients' safety, mobility, and functional capabilities within their home environments, adjusting treatment strategies to promote independence and effective rehabilitation.
• Educate patients and their families through detailed explanations of therapeutic exercises, body mechanics, and adaptive strategies for daily living.
• Utilize secure and user-friendly Electronic Medical Record (EMR) tools to document patient sessions accurately and maintain progress notes.
• Engage in interdisciplinary case reviews, contributing insights and learning from collective discussions to enhance patient care continuously.

Patient Population / Client Focus

You will work primarily with patients recovering from orthopedic surgeries, musculoskeletal injuries, and those managing chronic pain conditions. Your knowledge of their conditions will be critical in crafting personalized treatment plans that cater to their specific rehabilitation needs.

Qualifications

• Current physical therapy license in the State of New Jersey or eligibility for licensure.
• A bachelor's degree in physical therapy from an accredited institution.
• Strong interpersonal and organizational skills to effectively manage patient caseloads and foster communication.
• The ability to work independently while demonstrating flexibility in adapting to a variety of home environments.

Preferred Experience

• A minimum of two years of experience in homecare or outpatient orthopedic settings is preferred, although confident new graduate candidates will be considered.
