Key Responsibilities\n

• Conduct comprehensive behavioral health assessments and crisis evaluations, utilizing standardized cognitive and behavioral assessment tools to identify client needs accurately.

• Engage in individual and group psychotherapy, providing therapeutic support tailored to both patients and their families.

• Offer ongoing resources and support to caregivers, ensuring they receive the guidance needed to accompany clients on their recovery journey.

• Determine the types and lengths of services necessary to meet individual client needs, crafting personalized treatment plans.

• Facilitate telehealth consultations and provide telephone support services, ensuring accessibility for clients and their families.

• Coordinate care with primary care physicians, specialists, and home health aides, strengthening the support network around each patient.

• Manage case services effectively for both patients and their caregivers, ensuring seamless transitions and continuity of care.

Qualifications\n

Compensation and Benefits\n
