Key Responsibilities\n

• Facilitate educational sessions focusing on chemical dependency, equipping patients with knowledge and coping strategies.

• Provide individual and group counseling focused on chemical dependency, utilizing evidence-based practices to promote recovery and resilience.

• Offer therapeutic support through family, adolescent, and multi-family therapy sessions as needed, fostering an inclusive environment for healing.

• Deliver case management services by coordinating resources and referrals to support patient needs and ensure access to essential services.

Patient Population / Client Focus\n

You will primarily work with patients facing terminal illnesses and their families, providing emotional, psychological, and logistical support. Qualifications\n
