Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to determine the needs of children and families.

• Develop and participate actively in the planning and implementation of Individualized Family Service Plans (IFSPs).

• Monitor and document outcomes, contributing to the continuous improvement of services provided.

• Provide support and guidance to families, equipping them with resources and strategies that promote their child’s development.

• Adhere to all Federal and State regulations and ethical standards while performing social work duties.

• Foster positive relationships with clients, families, and colleagues to promote a collaborative working atmosphere.

• Utilize the coaching model and natural environment teaching to enhance learning experiences for families.

• Collect feedback and implement alterations in response to individual family needs and data-driven outcomes.

Patient Population / Client Focus\n

Your primary responsibility will be working with young children, particularly those with disabilities or developmental delays, and their families. This role focuses on supporting children in reaching critical developmental milestones during their formative years, specifically from birth to three years old. Understanding the child’s unique context and working closely with their families will be vital for successful intervention and support.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Active LCSW licensure from the Department of Public Health.
\n
• A valid driver’s license.
\n
• Demonstrated commitment to diverse learning styles to effectively meet individual child and family needs.

• Strong organizational and time management skills to handle multiple cases efficiently.

• Exceptional communication skills, both verbal and written, to facilitate effective collaboration.

Preferred Experience\n

Compensation and Benefits\n
