Forest Manor Rehabilitation Services is seeking a dedicated and compassionate Physical Therapist to join our team at our state-of-the-art rehabilitation center in San Francisco, California. This full-time, onsite position offers a competitive salary range of USD 90,000 – 110,000 annually, along with a comprehensive benefits package. Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing relevant medical histories, performing specific assessments, and developing individualized treatment plans based on findings.
• Communicate evaluation results and treatment strategies effectively to patients and their families, involving them in goal-setting and ongoing care discussions.
• Implement treatment protocols proficiently, tailoring approaches based on the progression of the patient's condition and the guidelines of contemporary healthcare practices.
• Collaborate with other healthcare professionals to ensure a holistic approach to patient care, documenting necessary changes to the treatment plan as patients' needs evolve.
• Manage discharge planning and follow-up care, ensuring accurate and timely documentation that reflects the entirety of the patient's rehabilitative journey.
• Demonstrate effective time management skills to meet productivity expectations and maintain accurate patient care schedules.
• Actively participate in departmental goals and initiatives, contributing to compliance with regulations and accreditation standards.
• Engage in continuous professional development by pursuing educational opportunities and serving as a clinical instructor for students.
• Promote a patient-centric environment by advocating for excellent customer service and accessibility to care.

Patient Population / Client Focus

You will primarily serve adults recovering from orthopedic surgeries, sports injuries, and neurological conditions. Our patient population is diverse, including individuals seeking rehabilitation after accidents, chronic pain management, and post-operative care. You will be instrumental in helping these individuals regain mobility, strength, and independence through customized rehabilitation programs.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• A Doctorate in Physical Therapy is preferred.
• Current state licensure as required for practicing physical therapy in California.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• No prior work experience is required, making this an excellent opportunity for newly graduated physical therapists.

Preferred Experience

While prior experience is advantageous, it is not mandatory. New graduates are encouraged to apply, as we provide ongoing mentorship and support to build your expertise in an outpatient rehabilitation setting. Familiarity with electronic health record (EHR) systems is beneficial and will facilitate efficient documentation and communication.
