Work Environment

Key Responsibilities

• Conduct thorough assessments to identify and set patient-specific goals relating to mobility and health improvement.
• Create and implement individualized treatment plans that utilize a variety of intervention techniques tailored to each patient's condition.
• Handle a patient caseload with an expected frequency of one individual per hour, ensuring each session is engaging and focused on achieving measurable outcomes.
• Utilize evidence-based practice to deliver high-quality care that aims to maximize patient independence and improve overall function.
• Facilitate the reduction of pain and mobility limitations through manual therapy techniques and therapeutic exercises.
• Motivate patients throughout their treatment process and monitor progress, providing feedback to maintain engagement and encourage adherence.
• Document all patient care services provided meticulously, ensuring compliance with health regulations and organizational standards.
• Collaborate with multiple healthcare professionals to provide well-rounded patient services and consult on complex cases.
• Perform regular evaluations of treatment progress and modify care strategies as needed to align with changing patient goals.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will work primarily with patients recovering from orthopedic surgeries, injuries sustained from falls, sports-related injuries, and chronic pain conditions. Your understanding of orthopedic rehabilitation and experience in managing diverse patient scenarios will be vital in fostering patient recovery and ensuring they return to their daily activities with confidence.

Qualifications

• A Master's degree or Doctorate (DPT) in Physical Therapy from an accredited institution.
• A valid state license to practice as a Physical Therapist.
• A clear professional record with no history of board or disciplinary actions against your license.
• Strong proficiency in advanced manual therapy techniques essential for effective patient outcomes.
• Exceptional competence in developing, implementing, and adapting exercise protocols based on best practices.
• Proficient in utilizing EMR Software, particularly WebPT, for accurate documentation and patient tracking.

Preferred Experience

• Prior experience working specifically in orthopedic outpatient settings is highly desirable.
• Demonstrated success in managing a diverse patient caseload with varying levels of complexity.
• Experience in developing fitness and wellness programs tailored to individual patient needs.

Compensation and Benefits

• A competitive annual salary ranging from $90,000 to $120,000, commensurate with experience and qualifications.
