Work Environment\n

This position is primarily remote, which allows for flexibility in scheduling to meet the needs of our clients. While our Social Worker will conduct assessments and provide support from home, there will be regular, in-person visits to clients' homes or care settings as necessary. The work hours are traditionally set from Monday to Friday, 8:00 AM to 4:30 PM; however, flexibility is crucial to accommodate client visits and address emergent situations, which may occasionally involve evening or weekend hours.

Key Responsibilities\n

• Perform comprehensive assessments of seniors’ psychosocial needs, taking into account their emotional, mental, and social circumstances.

• Collaborate with seniors and their families to develop personalized care plans that prioritize their unique goals and preferences.

• Provide counseling and emotional support, guiding clients through life transitions such as health changes, loss, or end-of-life issues.

• Act as an advocate for clients by connecting them with community resources, services, and support systems that can enhance their quality of life.

• Work closely with interdisciplinary teams, including healthcare professionals, caregivers, and nurses, to effectively coordinate care and address client needs.

• Conduct regular follow-up visits to maintain an open line of communication, monitor client well-being, and resolve any logistical or safety issues.

• Facilitate discussions among family members to resolve conflicts and ensure that everyone is aligned in the care approach for the senior client.

• Educate clients and their families about available programs and community support to enhance social engagement and a sense of belonging.

• Maintain precise, up-to-date documentation of client assessments, care plans, and interactions to ensure compliance with industry standards and facilitate continuity of care.

Compensation and Benefits\n
