Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients' mental health conditions and identify resources available to them.

• Provide individual, family, and group therapy using evidence-based modalities such as cognitive-behavioral therapy (CBT) and trauma-focused therapy.

• Develop and implement individualized treatment plans that align with clients' goals, regularly updating these plans based on progress and feedback.

• Collaborate closely with other healthcare professionals, including psychiatrists and psychologists, to coordinate care for clients effectively.

• Facilitate therapeutic group sessions and support groups that address common challenges faced by clients in hospice care.

• Maintain accurate documentation of patient interactions in compliance with legal and ethical standards, utilizing electronic health record (EHR) systems.

• Provide crisis intervention and safety planning for patients exhibiting high-risk behaviors.

• Engage in psychoeducational efforts, helping clients and their families understand and manage mental health issues related to terminal illness.

Patient Population / Client Focus\n

This role involves working primarily with patients who are part of our hospice program, which means you will encounter individuals from various backgrounds, including those facing terminal illnesses. A significant aspect of your work will involve supporting families through their grief journey, offering resources and emotional support to help them cope with the expected bereavement experience. You will also provide guidance on end-of-life decisions and facilitate discussions about advance care planning in a sensitive manner.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.

Compensation and Benefits\n
