Work Environment\n

This role is primarily remote, with the possibility of flexible in-home visits required to assess and support clients directly. You will collaborate with an interdisciplinary team, including healthcare professionals, nurses, and caregivers, to ensure coordinated care that meets the evolving needs of each senior. Your schedule will typically run Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend responsibilities based on client needs. You will be expected to be on-call as necessary to address urgent situations, providing critical support at any time.

• Conducting in-depth assessments of seniors to evaluate their psychosocial needs, taking into account emotional, mental, and social factors impacting their lives.

• Providing empathetic counseling and emotional support to assist clients and families during significant life transitions, such as health changes or mobility challenges.

• Advocating for clients’ rights and needs, including connecting them to essential community resources and navigating healthcare services.

• Working closely with a team of nursing staff and other healthcare professionals to deliver comprehensive and continuous care to our clients.

• Regularly visiting clients to monitor their conditions, assess their living environments, and identify any concerns regarding safety or well-being.

• Facilitating constructive communication among family members to resolve conflicts and ensure a cohesive care strategy.

• Educating clients and their families about available services, support groups, and programs that foster greater social engagement.

• Maintaining detailed and accurate records, including client assessments, care plans, and progression notes, using electronic health record systems.

• Staying abreast of current best practices in social work, particularly as they apply to aging, to ensure adherence to ethical standards and regulatory compliance.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited program.

Compensation and Benefits\n

We offer a competitive salary ranging from $60,000 to $100,000 per year, commensurate with experience and credentials.
