Key Responsibilities\n

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues within the clinic.

• Conduct comprehensive patient intakes, including diagnostic and functional evaluations, and develop appropriate treatment recommendations.

• Advise primary care providers on optimal referral paths for patients needing specialized mental health support, ensuring timely access to necessary resources.

• Initiate and maintain follow-up communications with patients to track progress, adjust treatment plans, and reinforce coping strategies.

• Create and oversee relapse prevention plans tailored to assist patients in achieving stable and healthy functioning.

• Identify high-risk patients, employing comprehensive assessment techniques to develop preventative strategies for managing their conditions.

• Document patient progress rigorously in treatment charts, maintaining clear and accurate records that inform future care decisions.

• Collaborate closely with primary care teams to ensure a holistic approach to patient treatment and recovery.

• Empower patients through education about self-management skills and encourage their active participation in the recovery process.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from $87,000 to $105,000 annually, commensurate with experience and qualifications. In addition to competitive pay, we offer a comprehensive benefits package that includes health insurance, a retirement savings plan, and opportunities for continuing education.
