Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing actively to the primary care team.

• Conduct comprehensive patient intakes, perform diagnostic and functional evaluations, and provide treatment recommendations tailored to individual needs.

• Implement brief and focused therapeutic interventions for patients requiring mental health support, ensuring timely access to necessary services.

• Evaluate patient needs to determine proper referrals to specialty mental health facilities as appropriate, effectively managing the integration of care.

• Monitor patient progress following initial assessments, adjusting treatment plans based on outcomes and feedback, thereby enhancing therapeutic effectiveness.

• Develop relapse prevention plans that assist patients in maintaining stability and promote long-term wellness strategies.

• Engage in outreach and preventive measures for identifying “at risk” patients and creating actionable plans to reduce the likelihood of deterioration in their conditions.

• Document all patient interactions meticulously in treatment charts, ensuring compliance with agency guidelines and promoting continuity in care.

• Work collaboratively with the care team to facilitate patient education and self-management strategies that empower individuals in their recovery journey.

• Stay abreast of current best practices in mental health care, providing insights and recommendations to clinic management regarding service improvements and outcomes evaluation.

• Comply with the Code of Ethical Conduct for Clinical Professionals and carry out any additional duties assigned within the scope of practice.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a closely related field is required.

• A current Licensed Professional Counselor (LPC) license in the state of Oregon is mandatory.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
