Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of clients' psychosocial needs to formulate effective treatment plans.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring that interventions are tailored to meet the unique needs of each client.

• Monitor and document client progress, adjusting treatment strategies as necessary to achieve desired outcomes.

• Provide emotional support and counseling to seniors and their families, facilitating discussions about end-of-life decisions, coping strategies, and resources available in the community.

Qualifications\n
