Key Responsibilities\n

As a Licensed Clinical Social Worker, your contributions are vital to our patients' well-being. Your daily responsibilities will include:

• Conducting thorough assessments to identify the unique counseling needs of each client, including utilizing DSM criteria to evaluate substance abuse diagnoses.

• Preparing tailored treatment plans that address substance abuse issues while considering the clients' overall mental health.
\n
• Developing comprehensive discharge and continuum of care plans to guide patients towards the appropriate resources post-treatment.
\n
• Documenting all counseling activities, including assessments, referrals, and follow-ups in each client's clinical record to maintain accurate and up-to-date information.
\n
• Maintaining a manageable caseload and ensuring timely updates for case management and utilization reviews.
\n
• Coordinating necessary support services for clients and their families, effectively communicating the resources available to enhance treatment outcomes.

• Engaging with referral sources, such as legal, educational, and employment systems, to facilitate a well-rounded support network for clients.

• Utilizing a multi-disciplinary team approach to address crisis situations, providing timely interventions to stabilize clients in need.

Patient Population / Client Focus\n

Your work directly impacts patients facing the challenges related to hospice care as well as those with co-occurring disorders. Your expertise in addiction counseling will allow you to guide clients through sensitive discussions and interventions aimed at promoting long-term recovery and enhancing overall quality of life during a crucial time.

Qualifications\n

To excel in this role, candidates should possess:

• A Master's degree in Social Work and a current New York State LCSW license.

This position offers a competitive salary range of USD 70,000 – 100,000 annually, complemented by a robust benefits package that includes:
