Key Responsibilities\n

• Conduct ongoing assessments with clients to determine their mental health needs, ensuring tailored treatment is provided.

• Maintain communication with supervisors, direct care staff, family members, and other stakeholders regarding client progress and treatment plans.

• Develop and share a tentative schedule of appointments, including follow-up home visits and observations to monitor the advancement of individual cases.
\n
• Facilitate and coordinate home visits to gather comprehensive data through chart reviews, staff interviews, and thorough data analysis.
\n
• Ensure the effective implementation of individual program plans, adjusting strategies as necessary to meet clients' goals.
\n
• Participate actively in Interdisciplinary Team Meetings, sharing insights and contributing to collaborative care approaches.
\n
• Prepare and complete monthly and quarterly progress notes, documenting treatment outcomes and client developments.
\n
• Organize and facilitate training sessions to ensure all team members are well-versed in the program plan and related documentation requirements.
\n
• Provide ongoing support through follow-up interviews and home visits, continuing to assess and address client needs.
\n
• Maintain accurate records to ensure transparency and share relevant information with authorized parties, acting as a valuable liaison with the mental health community.

• Collaborate in developing training materials and new procedures to enhance program delivery.

Qualifications\n

Compensation and Benefits\n
