Key Responsibilities\n

• Conduct comprehensive initial assessments and ongoing evaluations to tailor treatment plans that address the individual needs of each client.

• Maintain regular communication with supervisors, family members, and relevant stakeholders regarding patient progress and treatment adjustments.

• Develop and provide a structured schedule of appointments, home visits, and follow-up communications to monitor client advancement effectively.
\n
• Coordinate in-home observations, staff interviews, and data collection to ensure thorough understanding and documentation of client situations.

• Implement and oversee therapeutic programs, ensuring that they are effective and consistently delivered, adjusting as necessary based on client needs.

• Participate actively in interdisciplinary team meetings, providing insights and collaboration on patient care strategies.

• Document monthly and quarterly progress notes to track client development and adherence to their treatment plans.
\n
• Facilitate training sessions for staff and team members to ensure adherence to program implementation and documentation standards.

• Conduct follow-up interviews and home visits for ongoing evaluation and support of client's evolving needs.

• Act as a liaison between clients and the mental health community to foster partnerships and resource sharing.

• Help develop training materials and procedural documents aimed at enhancing program effectiveness.

Patient Population / Client Focus\n

The LPC will work with a diverse clientele, including adults and youth facing a variety of mental health challenges. These may range from anxiety and depression to more complex issues such as trauma, substance misuse, and family dynamics. Your ability to engage with clients from different backgrounds and tailor therapeutic approaches accordingly will be key to their progress and recovery.

Qualifications\n

• A Master's degree or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted licensure as a Professional Counselor in the state of Arizona.

• A minimum of 2 years of experience in counseling, particularly in clinical settings.

• Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly preferred.

• Strong understanding and adherence to legal and ethical standards in counseling practices.

• Demonstrated excellent interpersonal skills to build rapport with clients and collaborate effectively with team members.

• Proficiency in EMR software and related technologies for documentation and communication.

Preferred Experience\n

While not mandatory, candidates with experience in trauma-informed care, crisis intervention, and evidence-based therapeutic practices will be favored as these align closely with our program goals and enhance client outcomes.
