All Services And Care Are Provided In Accordance With Federal and State laws and regulations Centers for Medicare & Medicaid Services (CMS) requirements California Department of Public Health (CDPH) regulations Masonic Homes policies and procedures Professional standards of nursing and healthcare practice Resident Rights and Person-Centered Care principles HIPAA Privacy and Security requirements Employee Handbook policies and organizational values Position Summary The Skilled Nursing Facility Administrator serves as the licensed executive leader and administrator of the Lorber Care Center, a 125-bed licensed Skilled Nursing Facility providing skilled nursing, post-acute rehabilitation, long-term custodial care, and dementia care services. The Administrator is responsible for the overall leadership, strategic direction, regulatory compliance, financial performance, quality outcomes, resident satisfaction, physician relations, workforce engagement, risk management, survey readiness, and day-to-day operations of the facility. This position maintains 24-hour accountability for ensuring the delivery of safe, compassionate, high-quality care while maintaining compliance with all applicable federal, state, and local regulations. The Administrator serves as the facility's designated representative to regulatory agencies, including the Centers for Medicare & Medicaid Services (CMS), California Department of Public Health (CDPH), Office of Statewide Health Planning and Development (OSHPD/HCAI), local fire and safety authorities, and other governmental oversight organizations.ORGANIZATIONAL RELATIONSHIPS
The Administrator reports directly to the Chief Executive Officer or designee and collaborates closely with executive leadership, program directors, medical staff, consultants, regulatory agencies, residents, families, and community partners. Direct Reports Include Director of Nursing Services Health Information Manager Social Services Director Activities Director Staffing and Scheduling Coordinator Administrative Assistant Other department leaders as assigned Essential Responsibilities Provide overall leadership and operational oversight for all aspects of the Skilled Nursing Facility. Direct and evaluate all facility operations to ensure excellence in resident care, customer service, workforce engagement, operational efficiency, and regulatory compliance. Establish strategic goals and performance objectives aligned with organizational priorities. Foster a culture of accountability, resident-centered care, teamwork, and continuous improvement. Oversee implementation and monitoring of policies and procedures to ensure compliance with all applicable laws and regulations. Serve as the facility's primary representative with residents, families, physicians, consultants, regulatory authorities, and community stakeholders. Maintain a visible leadership presence throughout the facility and conduct regular leadership rounds. Lead emergency preparedness, disaster response, and business continuity planning. Performance may be evaluated using facility quality indicators, survey outcomes, resident satisfaction measures, census performance, workforce engagement metrics, regulatory compliance, and achievement of operational goals. Regulatory Compliance and Government Reporting The Administrator maintains overall responsibility for ensuring the facility's compliance with all federal and state reporting requirements, regulatory submissions, certifications, and survey processes. Responsibilities include: Oversee all CMS, CDPH, and other regulatory reporting requirements for Skilled Nursing Facilities. Ensure timely and accurate submission of required reports, including but not limited to: Minimum Data Set (MDS) reporting CMS Payroll-Based Journal (PBJ) submissions CASPER reports and Quality Measure reporting Nursing Home Care Compare data validation and compliance State staffing and workforce reporting Infection Prevention and Control reporting National Healthcare Safety Network (NHSN) reporting requirements Medi-Cal and Medicare participation requirements HCAI/OSHPD reporting requirements Annual licensure and certification filings Plan of Correction (POC) submissions Mandatory incident reporting Reporting of abuse, neglect, misappropriation, and unusual occurrences Emergency preparedness and life safety compliance reporting Maintain facility readiness for federal and state surveys, complaint investigations, certification reviews, and audits. Serve as the facility's primary liaison with CMS, CDPH, local regulatory agencies, and accreditation organizations. Monitor regulatory changes and ensure operational practices align with evolving requirements. Coordinate survey response activities and oversee implementation of corrective action plans. Ensure accurate documentation and record retention in accordance with regulatory requirements. Financial and Business Management The Administrator must ensure conformance with the Nursing Home annual budget. The incumbent investigates revenue sources, including grant writing, re-adjusts and re-directs services to maximize reimbursement and completes all required documentation to this effect. The incumbent also strives to maintain a high occupancy rate with payor sources to cover annual budget expenses. Participates on Medical Advisory Infection Control & Pharmacy Advisory Committees; confers with Medical Director and other Nursing Home consultants as necessary. Serves on various community boards to ensure continued care for residents. Monitor revenue, expenses, productivity, and financial performance against budget targets. Identify opportunities to maximize reimbursement and operational efficiency. Oversee census development and occupancy management initiatives. Collaborate with admissions, clinical, and finance teams to optimize payer mix and reimbursement strategies. Analyze operational and financial reports and recommend corrective actions when necessary. Develop business plans to support new programs, capital projects, and service enhancements. Present financial analyses, budget projections, and capital requests to executive leadership and the Board as appropriate. Human Resources and Workforce Leadership Provide leadership, coaching, and performance management to department leaders. Ensure staffing levels meet resident care needs while complying with state and federal staffing requirements. Promote employee engagement, retention, recognition, and professional development. Foster a positive workplace culture grounded in accountability, collaboration, respect, and inclusion. Address employee relations issues, employee grievances, and labor-management concerns. Support recruitment and retention initiatives to attract and retain high-performing professionals. Ensure compliance with labor laws, workplace safety regulations, and organizational policies. Clinical Operations Ensure the delivery of high-quality resident-centered care across all service areas. Monitor clinical outcomes, resident satisfaction, and quality performance indicators. Participate in admission and case review decisions, particularly for residents with complex clinical needs. Collaborate with physicians, advanced practice providers, consultants, and interdisciplinary teams to address resident needs. Evaluate care delivery systems and implement improvements to enhance resident outcomes. Support infection prevention, medication management, rehabilitation, dementia care, and specialty program initiatives. Respond to significant clinical incidents, emergencies, and resident concerns as needed. Conducts daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns. Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness and promote workplace safety and reduce employee injuries through effective prevention and risk-management practices. Quality Assurance and Performance Improvement (QAPI) Lead the facility's Quality Assurance and Performance Improvement (QAPI) Program. Chair or oversee Quality Assurance and Assessment Committee (QA&A) activities. Monitor quality indicators, survey trends, and performance metrics. Develop and implement corrective actions to improve clinical and operational outcomes. Establish quality improvement teams and facilitate system-wide improvement initiatives. Prepare annual quality management plans and reports. Ensure sustained survey readiness and continuous regulatory compliance. Oversee monitoring of infection prevention and control programs. Report quality initiatives and outcomes to executive leadership. Resident Experience And Resident Rights Promote a culture of dignity, respect, autonomy, and person-centered care. Ensure all residents receive services consistent with their goals, preferences, and plan of care. Protect resident rights and ensure compliance with all applicable resident protections. Address resident and family concerns promptly and effectively. Foster an environment that supports independence, meaningful engagement, and quality of life. Ensure confidentiality of resident information and compliance with HIPAA requirements. Safety, Risk Management, and Environment of Care Direct facility-wide safety and risk management programs. Ensure compliance with OSHA, CMS, CDPH, and Life Safety Code requirements. Oversee fire safety, emergency preparedness, disaster planning, infection prevention, and workplace safety programs. Conduct regular facility inspections and environmental rounds. Implement strategies to reduce employee injuries, resident accidents, and organizational risk exposure. Knowledge, Skills, And Abilities Thorough knowledge of federal and California regulations governing Skilled Nursing Facilities. Demonstrated expertise in CMS compliance, survey management, reimbursement systems, and quality reporting. Strong knowledge of long-term care operations, clinical practices, staffing management, and financial administration. Ability to lead large interdisciplinary teams and foster organizational excellence. Strong analytical, problem-solving, and decision-making skills. Excellent oral and written communication skills. Ability to manage multiple priorities in a complex healthcare environment. Experience with regulatory surveys, complaint investigations, and corrective action implementation. Ability to interpret financial, regulatory, legal, and operational data. Commitment to resident-centered care and organizational values. The Administrator maintains contact with the California Department of Public Health and with regard to licensure requirements, complaints, inspections, etc. RequiredEDUCATION, LICENSURE, AND EXPERIENCE
Current and unrestricted California Nursing Home Administrator (NHA) license. Minimum three (3) years of progressive leadership experience in Skilled Nursing Facility administration. Any employment offer is contingent upon completion of a background review consistent with California Fair Chance Act requirements and applicable healthcare licensing regulations. Demonstrated experience managing regulatory compliance, survey readiness, budgeting, staffing, and quality programs. Knowledge of CMS, Medicare, Medicaid (Medi-Cal), and California Department of Public Health requirements. Minimum requirements necessary to effectively carry out position responsibilities requires the incumbent to be licensed as a Nursing Home Administrator in the State of California and have extensive experience in administration of a long-term care facility; considerable knowledge of Federal and State licensure requirements for Nursing Home operations, and respect for the principles of resident rights, confidentiality, EEO and ADA. Preferred Bachelor's degree in Healthcare Administration, Business Administration, Public Health, Gerontology, or related field. Master's degree in Healthcare Administration, Business Administration, Public Health, or related discipline. Experience leading post-acute, long-term care, or continuing care retirement community operations.
