Key Responsibilities

Your responsibilities will encompass the full spectrum of patient care in a home care environment. Key tasks include:

• Evaluating patients referred by their physicians, developing treatment plans, and implementing care tailored to their specific needs and conditions.
• Utilizing specialized techniques and modalities to help patients regain function and improve their health status.
• Providing patient education to enhance understanding of their conditions and the importance of adherence to their treatment plans.
• Maintaining detailed documentation of patient progress and care plans to facilitate optimal recovery outcomes.
• Collaborating with a broader team of healthcare providers to optimize patient care and ensure the best treatment results.
• Meeting predefined productivity targets and participating in continuous quality improvement initiatives.

Patient Population / Client Focus

In this role, you will work with a diverse range of patients, including those recovering from orthopedic injuries, pediatric cases, neurologic conditions, and sports-related issues. Each patient presents unique challenges, allowing you to apply your expertise and creativity to inspire positive outcomes. Your ability to build rapport with patients and their families will be crucial in guiding them through their recovery process.

Qualifications

We are looking for candidates who possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid physical therapy license in the State of New Jersey.
• Certification in Basic Life Support (BLS) by the American Heart Association.
• Strong verbal and written communication skills to articulate treatment plans effectively.
• Proficient computer skills to manage electronic health records and other necessary tools.
• A passion for patient care, with the ability to establish and maintain strong therapeutic relationships.

Preferred Experience

A minimum of two years of clinical experience as a physical therapist is required. Experience working in home health or outpatient settings is highly desirable. Familiarity with evidence-based treatment methodologies, as well as a proactive approach to patient interaction, is also beneficial.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to USD 120,000 annually, along with a comprehensive benefits package that includes:
