Work Environment

As a member of our team, you will work onsite, providing therapy in a home care setting. The role requires you to visit clients in their residences, assessing their needs, implementing treatment plans, and ensuring a comfortable environment where healing and recovery can flourish. Key Responsibilities

Your responsibilities will include but are not limited to:

• Developing and implementing individualized treatment plans aimed at addressing mobility, strength, and functional capabilities specific to each client's needs.
• Directing physical therapy treatments that aim to prevent disabilities and relieve pain while restoring muscle function and promoting maximum performance.
• Overseeing physical therapy assistants to ensure they implement the prescribed plans of care effectively and safely.
• Conducting comprehensive evaluations using diagnostic tests to assess muscle, nerve, joint, and functional abilities.
• Adapting treatment strategies based on findings from home environment assessments to augment client's independence.
• Administering manual therapy, therapeutic exercises, and adjunctive treatments while educating clients and families on care protocols.
• Documenting progress and outcomes through clinical records such as initial evaluations, care plans, and discharge summaries.
• Participating in interdisciplinary team discussions and case conferences to collaboratively enhance the client's total treatment plan.
• Providing guidance on the use of adaptive equipment and prosthetics and consulting with the physician regarding any adjustments needed.
• Continuing to expand your own knowledge and skills through ongoing professional development and training opportunities.

Patient Population / Client Focus

The clients you will care for primarily include individuals recovering from surgeries, managing chronic illnesses, or needing rehabilitation following injuries. This role requires sensitivity to the emotional and physical needs of clients, many of whom may be elderly or dealing with complex health challenges. You will play a pivotal role in their rehabilitation journeys, helping them regain autonomy and confidence in their physical capabilities.

Qualifications

To be successful in this role, candidates should possess:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid New Jersey license to practice as a Physical Therapist.
• A minimum of 2 years of experience in physical therapy, ideally in home care settings that emphasize personalized treatment plans and holistic patient engagement.

Preferred Experience

While the above qualifications are mandatory, candidates with experience in home therapy or geriatric care will be highly regarded. Additionally, familiarity with electronic medical record (EMR) systems and strong communication skills are advantageous for this position.

Compensation and Benefits
