Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from orthopedic conditions, injuries, surgeries, or chronic pain, specifically in a home care setting.
• Develop personalized treatment plans utilizing a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and advanced modalities to alleviate pain and enhance movement.
• Implement and monitor treatment sessions, adjusting methods and goals as needed to ensure optimal patient progress.
• Document patient progress meticulously, tracking therapies administered, responses to treatment, and reassessing goals every step of the way.
• Educate patients, their families, and caregivers regarding treatment processes, self-care techniques, and effective use of assistive devices.
• Participate in continuous learning through workshops and professional networking, remaining current on advancements in physical therapy practices.

Patient Population / Client Focus

In this role, you will primarily work with individuals who are recovering from orthopedic surgery, sports-related injuries, and chronic musculoskeletal conditions. Given the nature of home care, you will encounter diverse patient demographics, providing you with the opportunity to tailor treatment plans according to unique needs and home environments, ensuring holistic recovery experiences.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• Active and valid State License in Physical Therapy.
• Demonstrated ability to communicate effectively, both verbally and in writing.
• Proficiency in word processing and the use of electronic health records.
• Flexibility and adaptability in meeting the varied needs of patients in a home setting.

Preferred Experience

• A minimum of two years of experience in physical therapy, with a focus on orthopedic cases preferred.
• Experience working in home care or community settings is advantageous but not required.

Compensation and Benefits
