Key Responsibilities

• Conduct thorough examinations and evaluations of patients to assess their physical abilities and limitations.
• Develop individualized treatment plans based on evaluation findings, ensuring clear treatment goals and assessments are in place.
• Provide consultations to other healthcare providers regarding patients' ongoing care and recovery processes.
• Engage with and educate patients and their families regarding treatment options and rehabilitation strategies.
• Document evaluation results, treatment interventions, and discharge plans while adhering to the highest standards of compliance and accuracy.
• Participate actively in community outreach initiatives, continuous quality improvement projects, and program development relevant to patient care.
• Maintain effective communication with physicians, referral sources, and multidisciplinary team members to coordinate optimal patient care.
• Supervise and mentor assigned support staff while managing departmental resources efficiently.
• Participate in staff meetings and engage in professional development through continuous education and training programs.

Qualifications
