Work Environment

The role is based onsite at our dynamic Rehabilitation Facility in Avenel, NJ, where you will work Monday through Friday. Key Responsibilities

• Conduct comprehensive evaluations to assess patients' medical histories, functional abilities, and specific needs, forming a basis for tailored treatment plans.
• Communicate effectively with patients and their families regarding evaluation findings, clearly articulating treatment objectives and involving them in setting goals.
• Implement evidence-based treatment protocols aligned with patient evaluations and individual health care requirements, adjusting as necessary based on ongoing assessments.
• Engage in interdisciplinary consultations with other healthcare professionals, recognizing when additional medical or rehabilitation input is needed, and document changes in patient conditions.
• Manage discharge planning, ensuring a smooth transition for patients out of therapy, accompanied by appropriate follow-up care and documentation.
• Maintain timely and precise documentation of patient encounters, progress notes, and discharge evaluations to promote clear communication and continuity of care.
• Practice efficient time management to meet productivity standards, ensuring that all patient care activities are completed within appropriate timelines.
• Participate actively in marketing initiatives within the hospital and community to bolster outreach and patient access.
• Contribute to departmental goals by complying with regulatory standards and actively pursuing professional growth through continuing education.
• Serve as a clinical instructor for students, fostering their development while enhancing your teaching and leadership skills.

Patient Population / Client Focus

You will primarily work with patients recovering from orthopedic injuries, chronic pain syndromes, and post-surgical rehabilitation. Our clinic promotes a patient-centered care model, where your role is essential in helping patients regain their mobility and quality of life. Qualifications

• Bachelor's degree in Physical Therapy. A Doctorate in Physical Therapy is preferred.
• Current state license to practice physical therapy in New Jersey.
• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers is required.

Preferred Experience

• While prior work experience is not mandatory, candidates with a background in orthopedic physical therapy or related rehabilitation fields will be given preference.

Compensation and Benefits
