Bright Path Social Work is seeking a dedicated and licensed Master Social Worker to provide comprehensive support to elderly clients at our community health clinic located in Tampa, Florida. Work Environment\n

The selected candidate will enjoy a full-time remote working arrangement with flexible scheduling. While the standard hours are Monday to Friday from 8:00 AM to 4:30 PM, the role may require occasional evening or weekend hours depending on client needs. The ability to adapt your schedule to ensure continuity of care is essential, as is being on-call for urgent situations that may arise.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial status to identify emotional, mental, and social needs that may affect their well-being.

• Collaborate closely with clients and their families to create tailored care plans that align with their individual goals and preferences, enhancing their quality of life.

• Provide empathetic counseling and emotional support to help seniors and their families navigate transitions such as relocation, health changes, or end-of-life issues.

• Advocate for clients' rights and facilitate access to essential community resources and services, ensuring they receive the support they need.

• Engage with interdisciplinary teams, including nurses, caregivers, and health professionals, to effectively coordinate and optimize care for clients.

• Make regular visits to clients' residences to monitor their well-being, assess living conditions, and identify any concerns or safety issues that need to be addressed.

• Foster effective communication among family members and mediate any conflicts that may arise in relation to seniors’ care.

• Educate clients and their families on available community resources, support groups, and programs that promote social engagement and overall well-being.

• Maintain thorough and accurate documentation of client interactions, assessments, and care plans in accordance with best practices and ethical standards.

• Stay current with ongoing changes in social work practices and regulations, ensuring compliance and continual improvement in service delivery.

Patient Population / Client Focus\n

This role primarily focuses on serving senior clients who may be grappling with various challenges related to aging, such as chronic health issues, mobility limitations, isolation, and mental health concerns. The social worker will play a vital role in assessing these challenges and providing appropriate interventions that promote dignity and quality of life for older adults.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of Florida.

• At least two years of active experience in a social work role, preferably within a community center or geriatric setting that focuses on senior care.

• Strong understanding of social work principles, aging-related issues, and available community resources for seniors.

• Excellent skills in assessment, counseling, and creating effective problem-solving strategies.

Compensation and Benefits\n

The position offers an attractive compensation package ranging from $60,000 to $100,000 per year, complemented by a comprehensive benefits package that includes medical, dental, and vision insurance.
