Key Responsibilities\n

• Conduct comprehensive evaluations and develop personalized treatment plans tailored to individual client needs.

• Consult on crisis intervention strategies and develop behavioral guidelines that address pre-crisis and crisis situations, risk assessments, positive behavior plans, and medication management practices.

• Maintain compliance with regulatory standards by ensuring timely and accurate documentation of client interactions and treatment outcomes.

• Utilize evidence-based screening tools recommended by the clinical team to evaluate and monitor client progress.

• Collaborate effectively with multidisciplinary teams to design and implement positive behavior support strategies that enhance client recovery.

• Act as a liaison for clients navigating services and systems across governmental agencies and community organizations, ensuring that their needs are met effectively.

• Engage actively in licensing and accreditation processes, contributing to the organization’s commitment to maintaining high standards of care.

• Establish and sustain clear communication channels with clients, team members, and external partners to achieve our community health goals.

Qualifications\n
