Key Responsibilities\n

As a Licensed Clinical Social Worker, your pivotal role will involve:

• Conducting comprehensive evaluations and assessments to determine the needs and strengths of children with disabilities or developmental delays.

• Participating actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring that family priorities and concerns shape personalized care strategies.

• Monitoring progress and outcomes as a vital member of a multidisciplinary team, facilitating communication and collaboration among all stakeholders.

• Providing emotional support and resources to families, equipping them with skills and strategies to foster their child's development.

• Utilizing evidence-based practices while adapting to each child's unique learning style during implementation of interventions.

Patient Population / Client Focus\n

This role primarily focuses on serving families with young children experiencing disabilities or developmental delays at our hospice center. Our children's services are essential in supporting the early intervention goals that allow infants and toddlers to develop critical skills in physical, cognitive, social, emotional, and communication areas. You will play a crucial role in ensuring that these children receive the support they need during their formative years, positively affecting their educational and life outcomes.

Qualifications\n

• A Master’s Degree in Social Work or a Doctorate of Social Work is required.

• A valid license from the Department of Public Health as a Licensed Clinical Social Worker (LCSW).
