Work Environment

This position offers the opportunity to work onsite, providing therapy within clients' homes. Key Responsibilities

• Conduct thorough evaluations to assess clients' physical abilities and limitations, using diagnostic tests and baseline measures to develop individualized treatment plans.
• Direct and implement comprehensive physical therapy interventions aimed at preventing disability, alleviating pain, and restoring function, all tailored to meet each client's unique needs.
• Oversee and guide Physical Therapy Assistants (PTAs) in executing the treatment plans effectively and ensuring high standards of care are adhered to.
• Administer manual therapy techniques, traction, and therapeutic exercises while demonstrating correct techniques to clients and their families.
• Regularly evaluate the home environment to identify modifications that may assist clients in achieving greater independence.
• Maintain meticulous clinical records including evaluations, progress notes, and discharge summaries, ensuring compliance with OASIS guidelines and timely submissions.
• Engage collaboratively with an interdisciplinary team in case conferences to review and optimize the overall treatment plan.
• Provide education and support to clients and families on managing health conditions and utilizing physical therapy devices effectively.
• Continuously monitor and document clients' progress, reporting any significant changes in their condition to physicians and case managers promptly.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Minimum of 2 years of relevant experience, ideally in both acute care and home care settings.
• Current state licensure as a Physical Therapist in good standing and CPR certification.
• Strong communication and interpersonal skills to effectively engage with clients, families, and the healthcare team.
• A valid driver's license and automobile insurance are necessary for travel to client homes.

Compensation and Benefits
