Work Environment\n

This position is a full-time remote role that offers flexibility while ensuring you maintain an integral part of a dynamic team dedicated to hospice care. You will work Monday to Friday from 9:00 AM to 5:00 PM, allowing you the balance of professional and personal life amidst your important work. Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments that are sensitive to the diverse backgrounds and needs of patients within the hospice care framework.

• Provide individual, couples, and family counseling, addressing a range of issues including grief, mental health challenges, and substance abuse, utilizing evidence-based therapeutic techniques.

• Develop and implement personalized treatment plans focused on crisis intervention, which may involve collaboration with other healthcare providers to ensure comprehensive patient care.

• Document clinical encounters meticulously in electronic records, maintaining confidentiality and complying with all regulatory standards.

• Serve as a liaison, connecting patients and families with community resources, including support services and medication assistance programs.

Your primary focus will be on patients receiving hospice care, which requires a deep understanding of the physical, emotional, and spiritual aspects of end-of-life care. Qualifications\n

Compensation and Benefits\n
