Work Environment\n

This position allows for remote work, providing flexibility while maintaining essential face-to-face interactions with clients through scheduled video calls or in-person visits when necessary. Our team works on a Monday to Friday schedule, with hours from 8:00 AM to 4:30 PM, though occasional evening or weekend hours may be required based on client needs. The on-call duties will involve providing support during urgent situations that may arise outside of regular working hours.

Key Responsibilities\n

• Conduct in-depth psychosocial assessments to evaluate the emotional, mental, and social factors impacting seniors' well-being.

• Develop personalized care plans in collaboration with clients and their families, considering their unique goals and preferences for care.

• Provide counseling sessions tailored to the individual needs of seniors, supporting them through transitions and personal challenges.

• Advocate for clients’ needs and rights, connecting them with necessary community resources and services.

• Work collaboratively with a multidisciplinary team that includes healthcare professionals, caregivers, and community organizations to coordinate comprehensive care.

• Regularly visit clients to monitor their health and living conditions, ensuring they receive the highest level of care and attention.

• Be a mediator in family discussions regarding care strategies, fostering communication and resolution of conflicts.

• Educate clients and families about social work resources, support groups, and programs that enrich their lives.

• Maintain accurate and thorough documentation of client progress, interventions, and needs in accordance with ethical guidelines.

• Stay abreast of evolving social work practices and policies to ensure compliance and enhance care delivery.

Qualifications\n

Compensation and Benefits\n
