Work Environment\n

This position is primarily remote, allowing you the flexibility to manage your schedule while being connected to our community health clinic in Scranton, PA. You will work collaboratively with a multidisciplinary team, including healthcare professionals, caregivers, and community resources, to provide continuous care. Our hours of operation are Monday through Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed based on client availability.

Key Responsibilities\n

As a Licensed Master Social Worker at Bright Star Counseling, your primary responsibilities will include:

• Conducting thorough assessments of seniors to evaluate their emotional, mental, and social needs.

• Developing individualized care plans in collaboration with clients and their families, addressing specific goals and preferences.
\n
• Providing counseling and emotional support to help seniors and their families navigate significant life transitions such as health changes, relocation, or loss.
\n
• Advocating for clients by navigating community resources and ensuring they receive the support services necessary for their well-being.
\n
• Coordinating with an interdisciplinary team, including medical professionals and caregivers, to enhance the overall care of our clients.
\n
• Regularly visiting clients to monitor their well-being, assess living conditions, and address any arising concerns.
\n
• Facilitating family communications to resolve conflicts and promote a unified approach to care.
\n
• Educating clients and families on available community resources to promote enhanced social engagement and overall quality of life.

• Maintaining detailed and up-to-date records of client assessments, care plans, and progress notes in compliance with industry standards.

• Staying informed on current social work practices, ethics, and regulatory changes to ensure best practices are followed.

Patient Population / Client Focus\n

Your primary focus will be on seniors, a demographic with distinct psychosocial challenges and changing needs. You will be expected to understand the complexities of aging, including mental health challenges, social isolation, and access to care, tailoring your approach to meet these clients on an individual level.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.

• A minimum of two years of professional experience in social work, preferably within a community center or geriatric care setting.

• Comprehensive knowledge of aging-related issues, social work principles, and community resource networks.

• Exceptional assessment, counseling, and problem-solving abilities.

• A compassionate demeanor with the ability to build rapport with seniors and their families.

• Strong written and verbal communication skills to ensure effective collaboration with team members.

Preferred Experience\n

While not mandatory, experience working with geriatric populations or in a community health setting is advantageous. Familiarity with local community resources and social service agencies will enhance your effectiveness in this role.
